After the codification of supported decision-making in Utah law, the DLC has received requests several requests from the public about how to draft an agreement and examples of agreements. After some research, we discovered some organizations in other states, including some other P&As, had created a template on their website to fill out that would produce an agreement to be signed. We then developed some information and instructions about supported decision-making agreements, and we developed a draft template. We then worked with our web developers to build those pages on our website. Those resources are now live on our website.
An individual can now use this form to create a supported decision-making agreement. They simply have to provide information about themselves, their supporters, and select what is included in the agreement. After completed the form, there will be an option to download the agreement and a copy will be emailed to the address they provided. They will have to then sign the agreement with either two witnesses or have it notarized, as is required in the law. The DLC will not receive any personal information when the form is utilized. We are very excited to share this resource, and we encourage others to share it with those who might be interested in creating a supported decision-making agreement.
